I have legal custody of the student I am submitting a registration for and have given my consent for her/him to attend programs/events being organized by Riverside Drama Company. I understand that there are inherent risks involved in any program/event, and I hereby release Riverside Drama, Riverside Children's Arts Center, Riverside Avenue Christian Church, and all of its employees, members, agents, and volunteers from any and all liability for any injury, loss, or damage to person or property that may occur during the course of my child's involvement.
In the event that my child is injured and requires medical attention, I consent to reasonable medical treatment as deemed necessary. in the event treatment is required from a physician and/or hospital personnel designated by Riverside Drama, I agree to hold such person free and harmless of any claims, demands, or suits for damages arising from the giving of such consent. I also acknowledge that as a legal parent or guardian, I will be ultimately responsible for the cost of any medical care should the cost of that medical care not be reimbursed by the health insurance provider.
Production Agreement:
This production agreement is designed to confirm the students and parent/guardians' responsibilities. Please read the agreement carefully and let us know if you have any questions.
The guardian and student agree to the following terms:
- To attend and participate in all rehearsals.
- To ensure the student arrives prior to the start time for all rehearsals and is ready to pick-up the child at the rehearsal end time (5:00 p.m. on after-school days).
- To allow Riverside Drama to use interviews, photos, audio, and videos taken during all auditions, rehearsals, and performances for promotional purposes via print, web, and social media.
- To keep all distractions such as phones, toys, technology, food, and drinks offstage and put away during rehearsals.
- To keep negative attitude and behaviors from interrupting rehearsals and performances.
- To keep a fun-loving, positive attitude onstage and enjoy the show!
Payment:
The cost of participating includes snacks at rehearsals and complimentary tickets to the production.
